Indiana Tobacco Prevention and Cessation
Smoke-free workplaces save lives and money
While the adverse health effects associated with smoking and
exposure to secondhand smoke are universally accepted, many business
owners still do not understand the impact tobacco use has on their
bottom lines.
It is estimated that Hoosiers spend $1.9 billion each year in
tobacco-related health-care costs. Add to that lost productivity,
increased absenteeism, more frequent cleanings and quickly deteriorating
equipment, and Indiana businesses are losing $3,400 per smoker each
year.
The most recent statistics rank Indiana 49th among the states in
providing smoke-free workplaces, but with ongoing progress in
communities across the state, an increasing percentage of the Hoosier
workforce will be protected from secondhand smoke. Bloomington passed
Indiana's most comprehensive smoke-free ordinance protecting all
workplaces including restaurants and bars. The Indianapolis Marion
County City Council passed an ordinance on May 23 to require all
workplaces including restaurants to become smoke-free by March 1, 2005.
Public health advocates continue to push for such protection from
Evansville to Angola.
Business owners can be leaders in protecting Indiana's
workforce as well as increasing profitability. In addition to
voluntarily making their own locations smoke-free, many business owners
are partnering with public health officials to support local legislation
making all area businesses smoke-free.
Data from many cities and individual companies that have eliminated
smoking from the workplace shows no adverse sales effects for businesses
like restaurants and bars. In fact, surveys have shown that a majority
of people intend to go out more often after smoke-free legislation is
passed.
In addition to protecting patrons and workers from secondhand smoke
and lowering costs, smoke-free workplace policies also help those
employees who want to quit smoking in their battles against nicotine
addiction. This creates a healthier pool of employees for all
businesses.

Prevention is the best medicine for healthy people
Thanks to savvy employers, preventative care has taken its rightful
place alongside treatment of injuries and illness for many of
today's workers. It's a shift in focus that benefits employees
and employers alike.
Taking preventative health-care measures means fewer injuries and
illnesses, work-related and otherwise. That translates into less pain
and lost wages for employees and more productivity and fewer claims for
employers. Helping northwest Indiana and northeast Illinois reap these
benefits is Community Healthcare System's Occupational Health
Services.
NEW STANDARDS OF SUCCESS
Located at St. Catherine Hospital in East Chicago and St. Mary
Medical Center in Hobart, Occupational Health Services is helping
businesses and their employees across the region identify new standards
of success. "As a member of the Northwest Indiana Business
Roundtable, I hear firsthand how proud companies are to report the
number of days their company has gone without an injury or OSHA
recordable," says regional salesperson for Occupational Health
Services Kelly Kernagis. "What I don't hear is how they
prevented a heart attack or stroke."
Heading off such medical catastrophes is desirable not only in the
interest of longevity, but also in considerable dollars saved. On
average, companies can save $5 in insurance costs for every $1 spent on
prevention.
These prevention efforts include a variety of wellness programs
employees can participate in voluntarily. In most cases, these services
are extended to employees' spouses, as well as retirees. Employees
can learn if they are at risk for high blood pressure, heart disease or
stroke.
"I think it's important for employees to know that their
results are confidential and mailed directly to them. The company has no
access to these records. It's not just a courtesy we provide,
it's the law" says Sylvia McKnight, M.D., a St. Mary Medical
Center occupational health physician.
RECOMMENDATIONS FOR HEALTH
Once a health appraisal is complete, this wellness program will
recommend lifestyle changes that may help workers avoid a catastrophic
event, such as a heart attack. Additionally, individuals are provided
with educational offerings, which are available throughout the Community
Healthcare System, focusing on specific concerns such as how to lower
cholesterol or lose weight. Many of these classes are free and others
have a nominal charge.
"We utilize collective information which is given to a company
in a pie chart or graph format, so it has a clear understanding of the
overall health of its workforce and potential risks" Kernagis says.
Often, this information can help companies prevent future catastrophic
losses. In some cases, human resources managers may be able to use it to
negotiate reduced insurance premiums for their employees.
Unfortunately, illness and injuries do occur even when the best
preventative measures are in place. The Occupational Health Services
program boasts injury numbers below national averages on lost workdays
and restricted duty for the four most common injuries treated in
occupational health. Because it is a hospital-based program, it has
immediate communication with the emergency department, which aids in
keeping those numbers down.

The numbers tell the story, and the numbers are staggering. Indiana
holds the dubious distinction of simultaneously ranking in the top 15 in
several categories of chronic disease and high-risk behavior and in the

THE PROBLEM
The most recently published numbers show that the nation's
health-care costs now exceed $1.7 trillion per year. Medical costs for
chronic diseases, most of which are caused by the type of high-risk
behavior typified by the rankings listed above, account for 60 percent
of these costs. Add in the incidence of violent behavior or behavior
that may cause injury (such as not wearing seat belts), and you start to
get the picture.
Frequently, even after working with a number of insurance brokers
or consulting firms, employers feel that they are being given
"cookie-cutter" solutions to their benefits dilemma. Many
companies feel that they have lost control as they try to cope with
constantly escalating costs and try to maneuver through the maze that
