Indiana Tobacco Prevention and Cessation

Smoke-free workplaces save lives and money

While the adverse health effects associated with smoking and exposure to secondhand smoke are universally accepted, many business owners still do not understand the impact tobacco use has on their bottom lines.

It is estimated that Hoosiers spend $1.9 billion each year in tobacco-related health-care costs. Add to that lost productivity, increased absenteeism, more frequent cleanings and quickly deteriorating equipment, and Indiana businesses are losing $3,400 per smoker each year.

The most recent statistics rank Indiana 49th among the states in providing smoke-free workplaces, but with ongoing progress in communities across the state, an increasing percentage of the Hoosier workforce will be protected from secondhand smoke. Bloomington passed Indiana's most comprehensive smoke-free ordinance protecting all workplaces including restaurants and bars. The Indianapolis Marion County City Council passed an ordinance on May 23 to require all workplaces including restaurants to become smoke-free by March 1, 2005. Public health advocates continue to push for such protection from Evansville to Angola.

Business owners can be leaders in protecting Indiana's workforce as well as increasing profitability. In addition to voluntarily making their own locations smoke-free, many business owners are partnering with public health officials to support local legislation making all area businesses smoke-free.

Data from many cities and individual companies that have eliminated smoking from the workplace shows no adverse sales effects for businesses like restaurants and bars. In fact, surveys have shown that a majority of people intend to go out more often after smoke-free legislation is passed.

In addition to protecting patrons and workers from secondhand smoke and lowering costs, smoke-free workplace policies also help those employees who want to quit smoking in their battles against nicotine addiction. This creates a healthier pool of employees for all businesses.

Indiana Tobacco Prevention and Cessation can provide more information and support for business owners and employees wanting to make their workplaces smoke-free. For more information, call 866-515-LIFE.

Community Healthcare System

Prevention is the best medicine for healthy people

Thanks to savvy employers, preventative care has taken its rightful place alongside treatment of injuries and illness for many of today's workers. It's a shift in focus that benefits employees and employers alike.

Taking preventative health-care measures means fewer injuries and illnesses, work-related and otherwise. That translates into less pain and lost wages for employees and more productivity and fewer claims for employers. Helping northwest Indiana and northeast Illinois reap these benefits is Community Healthcare System's Occupational Health Services.

NEW STANDARDS OF SUCCESS

Located at St. Catherine Hospital in East Chicago and St. Mary Medical Center in Hobart, Occupational Health Services is helping businesses and their employees across the region identify new standards of success. "As a member of the Northwest Indiana Business Roundtable, I hear firsthand how proud companies are to report the number of days their company has gone without an injury or OSHA recordable," says regional salesperson for Occupational Health Services Kelly Kernagis. "What I don't hear is how they prevented a heart attack or stroke."

Heading off such medical catastrophes is desirable not only in the interest of longevity, but also in considerable dollars saved. On average, companies can save $5 in insurance costs for every $1 spent on prevention.

These prevention efforts include a variety of wellness programs employees can participate in voluntarily. In most cases, these services are extended to employees' spouses, as well as retirees. Employees can learn if they are at risk for high blood pressure, heart disease or stroke.

"I think it's important for employees to know that their results are confidential and mailed directly to them. The company has no access to these records. It's not just a courtesy we provide, it's the law" says Sylvia McKnight, M.D., a St. Mary Medical Center occupational health physician.

RECOMMENDATIONS FOR HEALTH

Once a health appraisal is complete, this wellness program will recommend lifestyle changes that may help workers avoid a catastrophic event, such as a heart attack. Additionally, individuals are provided with educational offerings, which are available throughout the Community Healthcare System, focusing on specific concerns such as how to lower cholesterol or lose weight. Many of these classes are free and others have a nominal charge.

"We utilize collective information which is given to a company in a pie chart or graph format, so it has a clear understanding of the overall health of its workforce and potential risks" Kernagis says. Often, this information can help companies prevent future catastrophic losses. In some cases, human resources managers may be able to use it to negotiate reduced insurance premiums for their employees.

Unfortunately, illness and injuries do occur even when the best preventative measures are in place. The Occupational Health Services program boasts injury numbers below national averages on lost workdays and restricted duty for the four most common injuries treated in occupational health. Because it is a hospital-based program, it has immediate communication with the emergency department, which aids in keeping those numbers down.

CLS Benefit Solutions Inc.

Real incentives to change high-risk behaviors

The numbers tell the story, and the numbers are staggering. Indiana holds the dubious distinction of simultaneously ranking in the top 15 in several categories of chronic disease and high-risk behavior and in the bottom 15 in identification and prevention activities for a number of health problems.

THE NUMBERS

Indiana's "Top 15" rankings:

* No. 1 in the number of fast-food restaurants per capita

* No. 5 in percentage who smoke

* No. 8 in percentage classified as obese

* No. 12 in percentage diagnosed with diabetes

* No. 15 in percentage diagnosed with heart disease

Indiana's "Bottom 15" rankings:

* No. 36 in those getting cholesterol checks

* No. 40 in those getting mammograms in the past two years

* No. 41 in those who have ever had a colon screening

* No. 44 in those getting annual physicals

And, although it's not a statistic that fits within the above categories, Indiana has one of the largest cardiology practices in the United States. The cardiologists are here for obvious reasons.

(Sources: Indiana State Dept. of Health Behavior Risk Factor Survey; Centers for Disease Control; Henry J. Kaiser Family Foundation)

THE PROBLEM

The most recently published numbers show that the nation's health-care costs now exceed $1.7 trillion per year. Medical costs for chronic diseases, most of which are caused by the type of high-risk behavior typified by the rankings listed above, account for 60 percent of these costs. Add in the incidence of violent behavior or behavior that may cause injury (such as not wearing seat belts), and you start to get the picture.

Frequently, even after working with a number of insurance brokers or consulting firms, employers feel that they are being given "cookie-cutter" solutions to their benefits dilemma. Many companies feel that they have lost control as they try to cope with constantly escalating costs and try to maneuver through the maze that the employee benefits situation has become. Employers are looking for a strategy that will help them move away from approaches like cost shifting, and allow them to deal with the cause of the problem. They are looking for a cure, not just a Band-Aid.

THE SOLUTION

The solution is for companies, both large and small, to avail themselves of the type of consumer-driven health programs that incorporate real incentives to change high-risk behaviors, combined with wellness initiatives, such as health screenings, stress and weight management programs, smoking cessation, targeted health education and disease management. These programs, when properly facilitated, promoted and administered through CLS Benefit Solutions, can truly modify behavior and empower people to make the right choices. Another initiative at CLS is our annual Health Care Conference, where various health-care experts share their expertise with participants. (This year's conference will be held on July 19, 2005 at the Indianapolis Marriott North.)

CLS Benefit Solutions has been guiding employers through the maze of employee benefits for more than 10 years. Our staff of experienced consultants/brokers and other professionals helps companies solve employee benefit problems by listening to--and understanding--employers' needs. We apply our knowledge and experience to develop creative, customized employee benefits packages that help employers control benefits costs, stay abreast of compliance requirements, improve employee morale and, ultimately, remain competitive.

Tobias Insurance Group

The right tools for difficult decisions

Common wisdom tells you that when you are sick, you go to a doctor who has the right tools to properly evaluate the problem, ensuring that the appropriate treatment is implemented.

Common wisdom should also tell you that when your health insurance plan is sick--costs are too high, administrative issues, PPO access, compliance issues (COBHA, HIPAA)--you should go to an agency that has the right tools to properly evaluate the problems and can assist you in determining the appropriate treatment plan.

BENEFIT TOOLS.

There are sophisticated tools available today that can help an agency properly evaluate the health of your health insurance plan. Benefit modeling is a process that evaluates changes in utilization of health-care services and changes in employee behavior when a change in benefits is made to a plan. This allows an employer to test out benefit changes and the impact on costs before making the final decisions.

Benefit benchmarking is another tool that can assist an employer in comparing its health insurance plan to the plans of other employers in their industry, region or by size of company. Benchmarking helps an employer to stay competitive with those companies that compete for the same quality employees. Changes in benefits may save costs, but if you lose valuable employees, is it a worthwhile change?

Internet-based enrollment and employee access isn't for every employer. But if you have multiple locations or shifts, providing a consistent benefit presentation to all employees, and 24/7 access to the benefit menu, can be a daunting challenge. That is when the Internet starts to deliver for your employees and your HR staff.

Don't all agents provide the same service?

NO! Just because an agency specializes in employee benefit plans does not mean that they have all of the tools needed to properly evaluate your needs, determine the appropriate solutions, and implement the changes smoothly. An agency should have wide access to a variety of insurance companies, products and services in order to identify and implement the appropriate changes. It is important to make informed decisions, but we have found it is equally important to have an agency that can assist you in communicating those changes to your employees, both in print and in employee meetings.

THE RIGHT COMBINATION.

Combining the right tools, with wide access to solutions, and the clout to negotiate the best deals allows the agent to help you make and implement the difficult decisions that need to be made today.

TOBIAS INSURANCE GROUP INC.

As one of Indiana's largest independent agencies, our clients rely on our advisory services to help them get the most for their employee benefit dollars. Call us today for more information.

Unified Group Services Inc.

Technology-driven program allows for life-saving intervention.

Unified Group Services, Inc. offers a program with an unparalleled degree of intervention that is taking wellness to the next level. Powered by Active Health Management's CareEngine[R] system, our patient information is compiled from a variety of sources on all covered lives and analyzed against up-to-date evidence-based standards. This system uncovers opportunities to improve patient care and avert potential medical errors. Once these patient issues are pinpointed, the CareEngine system generates an alert--called a Care Consideration--which is reviewed by a physician and then communicated to the treating physician via phone, fax or letter. The result is a level of intervention unmatched in the industry.

AWARE Informed, the disease management component, is also powered by the CareEngine and works on all covered lives. It differs from traditional disease management as it actively engages both physicians and members with targeted, patient-specific information that helps all parties make smarter, informed clinical decisions about member care. Nurses proactively discover potential issues that impact a wide range of chronic conditions.

UGS also offers a complete range of wellness programs, including on-site screenings and health-risk assessments which are used as a supplement to the CareEngine. These programs are customized for individual client needs.
